Mesna, a drug used in cancer treatment, works by reacting chemically with the toxic metabolites of ifosfamide, a chemotherapy drug. This reaction helps to detoxify these metabolites, making them less harmful. The first step in this detoxification process involves Mesna binding to 4-hydroxy-ifosfamide, forming a non-toxic compound called 4-sulfoethylthioifosfamide. This helps to protect the patient's cells from the harmful effects of ifosfamide.
